The following frequently-used forms are now available online for Bethany staff and members:
Building Use Request (Non-Member/Civic Organizations)
Building Use Request (Member/BUMC)
Request for Check or Payment (Internal)
To complete PDF forms on your computer:
1. Click on the link for the desired form to open, then download it to your computer.
2. Once downloaded, click on the Fill & Sign button (may be under Tools). This will open the Fill & Sign Tools dialog box.
3. Click on Add Text (in the Fill & Sign Tools dialog box) and position cursor where you want to type in your information.
4. Once you have completed your form, save and print or email to office.
